Referrals

To refer a potential service user to us, simply call the referrals line on 07917 456 995. We will arrange to visit and assess your client and advise of the many options available within our facilities. Alternatively, please email outline information to andrew.lillington@clearwatercare.co.uk

We have a comprehensive assessment process that is followed for all our referrals. We actively involve the service user where possible, their relatives and the professionals currently providing their care. We endeavour to complete an assessment within 48 hours. Where it is apparent that we are not able to offer a service that will meet the person’s needs, we will make this clear to the referrer  and, where we can, will redirect them to other competent /professional providers. We would only recommend services we believe to be of a standard commenserate to those at Clearwater Care Group. 

Where we believe that the assessment suggests that we can meet the individual’s needs, we will provide a copy of our detailed assessment along with a completed Care Fund Calculator (CFC) and an offer letter to the referrer within 72 hours. As part of each assessment we undertake a CFC to ensure we objectify our fee levels and give the referring Local Authority a cost benchmark comparison.

Whilst our assessments follow a consistent framework they are specific to the individual needs of the person and the expectations of the referrer. For example, our assessments will differ slightly in terms of residential care and Supported Living which takes into account the expectations of the service user.

Our assessment process includes a sensory evaluation which we have found to be especially useful when working with people with Autistic conditions. Gaining an understanding of an individual’s sensory needs is important in delivering a better quality of life for the potential service user.

We will also give guidance to alternative placements if it is felt that Clearwater's care programmes are not suitable for the person or there is a more suitable alternative.
